Copies of the following forms/documents are required to be submitted for ERDI Leadership level registrations.

In addition to a copy of the above listed paperwork, the ERDI General Liability / Express Assumption of Risk form (ERDI Standards and Procedures Manual Part 6), any evaluation forms, Medical Questionnaire and Physician’s Sign off form, and copy of relevant course exam must be retained at the TDI Facility or by the TDI Instructor Trainer. They do not need to be sent for processing.
