Copies of the following forms/documents are required to be submitted for SDI Leadership level registration.

Divemaster, Assistant Instructor, Open Water Scuba Diver Instructor Registrations:

In addition to a copy of the above listed paperwork, the SDI General Liability / Express Assumption of Risk form (SDI Standards and Procedures Manual Part 6), and any evaluation forms must be retained at the SDI Facility or by the SDI Instructor/Course Director or Instructor Trainer. They do not need to be sent for processing.

Specialty Instructor Upgrade (for both Assistant Instructors and Open Water Scuba Diving Instructors)

If the candidate completes a specialty instructor upgrade program with a qualified IT, a copy of the above listed paperwork plus the SDI General Liability / Express Assumption of Risk form (SDI Standards and Procedures Manual Part 6), any evaluation forms, and a copy of the diver specialty exam must be retained at the SDI Facility or by the SDI Course Director or Instructor Trainer. They do not need to be sent for processing.

NOTE: All Assistant Instructors that wish to teach a specialty, MUST complete a Specialty Instructor Upgrade Program with a active status SDI IT or Course Director.
