Copies of the following forms/documents are required to be submitted for TDI Leadership level registrations.
Divemaster

In addition to a copy of the above-listed paperwork, the TDI General Liability / Express Assumption of Risk form (TDI Standards and Procedures Manual Part 4), any evaluation forms, Medical Questionnaire and Physician’s Sign off form, and copy of relevant course exam must be retained at the TDI Facility or by the TDI Instructor. They do not need to be sent for processing.
